Program/Service Notes:
Provides home health services including skilled nursing, physical, speech and occupational therapy, home health aides, and medical social worker. Patient care is provided 7 days a week, 24 hours a day. An individual case manager who is a nurse or therapist is assigned to each patient. Training and support are provided to family members who want to participate in the patient's care. The program is Medicare Certified and Joint Commission Accredited.
